IMPORTANT:
Before your appointment, please be sure and complete your AMCS Application at MedXpress.FAA.gov
This is very important so we can complete your exam when you arrive.
After you complete your application, be sure to save your work, and you will get a confirmation number. Be sure and print or write down this confirmation number. You will need to provide this number at the time of your appointment so that I may complete your application.
This information will not go to the FAA until I complete my part of the application as well. So, if you are not certain or have questions about an item on the application, just put down your best answer, and ask me about those items during your exam. Then I can update/correct your application as needed.
It is very important to note that the medical history is asking if you have had any of the listed issues ever in your life, not just since your last application. So be sure and list all items you have ever had such as surgeries, medications, etc.
If this is your first application or you have not renewed in several years, then read the "New Applicants" link above.

If you have trouble and cannot complete your application first, then no worries. Just come in for your appointment and I will help you. However, that may delay me issuing your medical for a few days, since I am not able to issue your medical until you complete your AMCS application at FAA.Medxpress.gov.
